請有需求之家長填妥申請表中的基本資料後，向輔導室提出申請。
說明：
一、依據社團法人中華安得烈慈善協會 115 年 3 月 16 日安字第 2026031602 號函辦理。
二、該會成立於 2011 年 8 月，主要以「食物銀行」計畫，每月配送 1 箱食物箱的方式，幫助 15 歲(含)以下的弱勢家庭學子，獲得所需營養之主副食品。
三、旨揭膳糧食物箱申請方式：
(一)學校檢附符合援助資格孩童資料、相關文件及申請表，以郵寄方式寄送至該會評估。
(二)該會審查期間若有緊急食物需求，可申請「安心食物箱」應急；貴屬學生家庭倘有急難狀況，亦可申請急難救助。
(三)受助個案，由該協會定期檢討評估援助時程。
