說明:
一、為了解本市學生是否於戶籍資料註記「熟」記事及族別認同，以保障平埔族群學生身分認同權及文化權，規劃發展推動相關教育政策，特進行本調查。
二、有關戶籍資料註記「熟」記事簡要說明:
(一)日治時期戶口調查簿中平地原住民的種族註記為「熟」，例如西拉雅族。
(二)臺南市自 102 年 7 月 20 日起受理平埔族群個人戶籍資料註記「熟」記事。本項記事須由民眾主動向戶政單位申請。

四、檢附戶籍資料註記「熟」記事之戶口名簿樣本供參。
