報名資格：以委員就職日 113 年 4 月 1 日起算，凡年滿 12 歲、未滿 25 歲（自民國 89 年 4 月 1 日以後至民國 101 年 4 月 1 日以前出生者）之青少年。
報名方式： 113 年 1 月 17 日(星期三)前，備妥報名資料寄至「國立虎尾高級農工職業學校學務處」(632004 雲林縣虎尾鎮博愛路 65 號)，信封上請註明「教育部國民及學前教育署第 6 屆青少年諮詢會之青少年代表報名」（郵戳為憑）或以電子郵件寄至報名信箱（ study12@hwaivs.ylc.edu.tw ）。
遴選方式：第一階段資格初審、第二階段書面複審及第三階段面試決審。
