二、檢附旨案實施計畫 1 份(詳附件)，擇要說明如下：
(一)報名資格：公私立國中小學現任教師及實習教師。
(二)收件資訊：
１、收件期限： 112 年 12 月 18 日(星期一)至 113 年 1 月 3 日(星期三)止。
２、收件方式：於收件期限內，以書面方式將參選作品限時掛號郵寄至「106308 臺北市大安區和平東路一段 162 號國立臺灣師範大學公民教育與活動領導系葉彤小姐收，並同步完成線上報名。
３、紙本寄送以郵戳為憑；線上報名以表單送出時間為憑，逾期或繳交資料如有缺漏或不符規定，視為未完成報名程序，恕不予審查。
