說明：
一、本(111)年為肯定身心障礙者、身心障礙照顧者及身心障礙 者服務人員，長期於身心障礙領域付出之貢獻，並為社會 樹立正面激勵之典範，入選者將於國際身心障礙者日主場 活動進行表揚。
二、由各推薦單位依本計畫訂定之表揚標準與類別，以 A4 紙張 格式將推薦表、授權同意書、切結同意書及相關佐證資料 (如專業證照或證書、志工服務時數證明、優良事蹟照片或 其他足以證明優良事蹟之文件)依序裝訂成冊，並於 111 年 8 月 31 日(週三)前函送推薦人選之應備文件(郵寄者以送件日 郵戳為憑。
收件地址： 708 臺南市安平區永華路二段 6 號 7 樓，收件人：社會局身心障礙福利科 張社工師收，信封請 註明：申請 2022 身障日表揚徵選)，逾期恕不受理。
