一、旨揭實施要點及推薦表，逕至教育局網站（ http://boe.tn.edu.tw ）／社會教育科／文件下載參閱。
二、若有推薦參加教育部本年度「社會教育貢獻獎」之團體或個人案件，請於 107 年 7 月 6 日（星期五）前（以郵戳為憑）檢附相關表件，寄送本局社會教育科（ 73001 臺南市新營區民治路 36 號），俾利本局評審並擇優送件，逾期或資料不齊全者不予受理。
三、各推薦單位對受推薦者或團體參選之具體事蹟，應審慎查核，推薦內容應以書面載明下列事項：
(一)具體事實及證明（包括歷年曾獲表揚之事蹟）。
(二)登記或立案之證明文件影本。
(三)依時序詳敘顯著事蹟。
(四)推薦單位之評語及印信。
(五)完整佐證資料。
四、推薦表及申請推薦繳交之證明書文件等資料，不予退還。
